It is required that all forms and documents are completed and returned prior to the child’s admittance to the program. Your signature on the Parent Admission Policies & Financial Agreement indicates your knowledge and acceptance of our policies and procedures as shared in the Cardinal Clubhouse Handbook.

Waiting List:
